Requirements and Charges for Issuance of Certificates to MBBS Students-Graduates

| 4 | Attempt Certificate | Application, CNIC, All Professional Result Cards & Original Fee Bank Challan |
| 5 | Trancript and Other Certificates i.e. Dean’s Letter, MSPE, Clinical Rotation, Language | Application & Original Fee Bank Challan is mandatory and the office can demand any documents from the applicant if required. If the applicant is abroad then he/she will send his/her request through email. |
| 6 | Verification | The Applicant shall submit fee and Documents/Forms which are to be verified. |
| 7 | Verification through e-mail | The Applicant will send email and Documents/Forms which are to be verified. His/Her any relative will submit the required fee in bank, thereafter the office will precede. |
| 8 | Attestation of documents | Fee Original Bank Challan & Documents which are to be attested. |
Note: The office will not precede any application, if any required fee original bank challan is not attached. All requests will be made on fulfillment of all requirements. No Relaxation shall be provided in this regard.
